Complete Decongestive Therapy

What CDT includes

CDT has four pillars that work together:

  1. Manual Lymphatic Drainage (MLD)

    • Gentle, rhythmic skin-stretching technique.

    • Stimulates superficial lymph vessels, reroutes fluid to healthy pathways.

    • Always applied proximal to distal, then distal to proximal, to “clear” and “drain.”

  1. Compression Therapy

    • Multilayer short-stretch bandaging in intensive phase.

    • Compression garments (custom-fitted sleeves/stockings) in maintenance phase.

    • Prevents refilling of fluid, improves muscle pump efficiency.

  1. Exercise Therapy

    • Simple, low-impact exercises performed while wearing compression.

    • Activates muscle pump, enhances lymph transport.

    • Focused on flexibility, mobility, and endurance (e.g., walking, light resistance bands).

  1. Skin Care & Infection Prevention

    • Hydration and daily inspection to prevent cellulitis.

    • Avoid trauma, burns, insect bites, and fungal infections.

    • Teach patients meticulous nail and cuticle care, use of pH-balanced moisturizers.

    🔹 Phases of CDT

  1. Intensive (Decongestive) Phase

    • Daily or frequent sessions (2–5x/week) with a therapist.

    • Focus: maximum volume reduction.

    • Includes MLD, bandaging, exercises, and skin care.

    • Duration: usually 2–6 weeks, depending on severity.

  1. Maintenance Phase

    • Self-management with compression garments, home exercise, and ongoing skin care.

    • Patient may still receive MLD periodically.

    • Goal: maintain results achieved in intensive phase.

    • Lifelong commitment for chronic lymphedema patients.

🔹 Why CDT Works

  • It doesn’t “cure” lymphedema but restores functional balance.

  • Improves lymph time volume and transport capacity.

  • Prevents skin breakdown and fibrosis.

  • Enhances quality of life, mobility, and self-image.
